Here are screen shots of the Supreme Court of Illinois' Web Site on lawyer registrations. One is Barack's registration (top) and the other is his wife Michelle's (middle). And another is the photo of the registration of Barack Obama when he was a student in an Indonesian private school (bottom).
Barack failed to list his former name, but his wife didn't.
